Life Membership

The Board of Directors has approved a special Life Membership. To qualify, a member must be

• 75 years old on 31 December of the year of application, and
• a dues-paying member for the last five years.

For example, a member may apply for Life Membership in 2012 if he or
she will turn 75 on the last day of 2012 and has paid dues in all years
from 2008-2012.

If you qualify for Life Membership, fill out the Life Membership application (PDF),
or write the following information on a piece of paper:

• name
• address
• telephone Number
• date of birth
• e-mail address
